Lung Health Initiative (LHI)

A Partnership Between CCPSA, Centura, and You!

With the support of Centura Health, The Lung Health Initiative (LHI) is a program that seeks to optimize the pulmonary care of patients admitted with lung disease. The goal of this initiative is to increase functional status, quality of life, and decrease risk of readmission to the hospital.

Any patient discharged from St Anthony Hospital, St Anthony North Hospital, Porter Hospital, Parker Hospital, Castle Rock Hospital, Avista Hospital, or Longmont United Hospital who meets criteria will be enrolled in the LHI program.

This program includes early contact with our nurses, access to electronic communication with our team, and early outpatient follow-up with Critical Care, Pulmonary and Sleep Associates!

91% of patients referred to the Lung Health Initiative stayed out of the hospital!
