Additionally, please allow the Updox program to access the camera and microphone on your device for communication purposes. We look forward to being a part of your healthcare team. 

INFORMATION ON CORONAVIRUS/COVID-19 

 If you have a scheduled appointment and have a fever and/or new respiratory symptoms such as cough or shortness of breath, please CALL our office to discuss further BEFORE coming for your appointment.  If any serious or concerning symptoms, please call 911.  We are now offering telehealth visits for our patients.  Please speak with a CCPSA team member to schedule. All patients should complete their new patient paperwork online prior to their appointment. Telemedicine patients must complete their paperwork online. If you cannot complete your paperwork online, please arrive 30 minutes early for your appointment.
